You can reach the Plague Demon by taking multiple routes from the Labyrinth. Once you reach the stairs and go up; you will arrive in the Vision Chamber. This is a very large room populated with the Plague Demon and annoying flies that he will spawn.
Be careful if you get close to the demon; as it has a nasty poison. He does move slow and its possible to kite him. Killing him will allow you to turn in the scrolls he drops for Vision Quest.
