Silvermoon is located in Axe just south of the portals. You simply enter via the portal, drop the Iron Ore and Yttril at his feet, and say: "s, craft greataxe".

Congratulations! You now are the proud owner of an SGA. The exit is located at the fire hex behind Silvermoon.
